Plans are organized into metal tiers (bronze, silver, gold, and platinum) that trade off monthly premium against out-of-pocket cost at the time of care.

The feature that matters most for many households is the income-based premium tax credit, which reduces the monthly premium based on projected household income for the year. Open enrollment runs from November 1 through January 15 each year; outside that window, enrollment requires a qualifying life event such as job loss, marriage, divorce, or the birth of a child.

Private Off-Marketplace Plans

The same carriers that sell ACA marketplace plans often sell additional plans outside the exchange as well. These off-marketplace plans are never eligible for premium tax credits, so they typically make sense only for people whose income is too high to qualify for a subsidy in the first place, or who want a plan design not offered on the exchange in their area.

Faith-Based Health Sharing Programs

Health sharing ministries allow members to share one another's eligible medical expenses based on a common faith commitment and community lifestyle guidelines. These programs are not insurance, are exempt from ACA requirements including guaranteed issue for pre-existing conditions, and typically carry lower monthly contributions than ACA premiums for healthy members. In exchange, members give up the legal payment guarantees that come with a regulated insurance policy, which makes this option a better fit for some households than others.

A Special Case: Self-Employed and Gig Workers

Independent contractors, freelancers, and small business owners technically shop the same ACA marketplace as everyone else without employer coverage, but two details are specific to this group. First, the self-employed health insurance deduction allows qualifying individuals to deduct 100 percent of their premium from taxable income, which changes the real after-tax cost of a plan meaningfully. Second, ACA subsidy eligibility is based on projected net income after business expenses, which can be genuinely difficult to estimate accurately when revenue varies month to month, making this a group for whom getting the subsidy calculation right matters more than most.

Medicare and Medicare Supplement Coverage

For Texans 65 and older, original Medicare Parts A and B cover hospital and outpatient care but leave meaningful cost-sharing gaps, including deductibles and 20 percent coinsurance with no annual out-of-pocket maximum. Medicare Supplement, or Medigap, plans are standardized private policies that fill these specific gaps, while Medicare Advantage offers an alternative, network-based path that replaces original Medicare entirely. The choice between these two approaches typically comes down to how much someone values provider flexibility versus the potential for lower monthly costs.

COBRA and Short-Term Transitional Coverage

One additional pathway deserves mention for anyone in the middle of a job change: COBRA allows a former employee to continue their previous employer's exact group plan for up to 18 months by paying the full premium directly, without the employer's contribution. Because the premium is no longer subsidized, COBRA is almost always more expensive month to month than an ACA marketplace plan with a tax credit applied. It tends to make the most sense only when maintaining continuity with a specific provider mid-treatment matters more than the price difference, since switching plans can otherwise mean switching networks.

How to Decide Which Path Fits Your Situation

With six distinct pathways available, the right starting question is not “which plan is cheapest” but “which category actually applies to me.” Someone with employer coverage available rarely benefits from shopping the individual market. Someone whose income qualifies for a significant ACA subsidy is usually better served there than in a faith-based program or COBRA, regardless of how the monthly contribution compares on paper in isolation.
