Breathlessness Is Not a Sign of Getting Older. Here's What It Actually Means.

A man in his 50s climbs two flights of stairs and needs a minute at the top. He's been doing this for a couple of years. His wife has noticed. He hasn't, or rather, he has, but he's put it down to being less fit than he used to be. Not 30 anymore. This is just what happens.

It isn't.

Getting winded during extreme exertion is normal. Getting breathless after a short walk, a single flight of stairs, or a conversation that involves any physical effort is not. That line gets crossed quietly, without any dramatic moment to mark it, in a way that makes it easy to ignore. By the time most people sit across from a doctor about it, the condition causing it has been progressing for years.

A Pulmonologist is the specialist who works out what's actually happening when breathing becomes difficult, limited, or unpredictable. The causes span structural lung damage, airway inflammation, progressive scarring, and blood oxygen problems that don't announce themselves clearly. Getting the right one identified early is what determines how much function can be preserved.

COPD: The Diagnosis That Usually Arrives a Decade Late

Chronic obstructive pulmonary disease is one of the most common causes of progressive breathlessness, and one of the most underdiagnosed conditions in India. The numbers consistently suggest that most people with it don't know they have it.

It develops slowly. Lung function declines year on year. The early stages feel like mild breathlessness on exertion, a morning cough that seems unremarkable, slightly more mucus than before. People adapt without realising they're adapting. They take the lift. They stop the evening walks. Eventually they stop noticing they've stopped.

By the time a significant event brings someone to hospital, they've typically lost 40 to 50 percent of their lung function. That function doesn't come back.

COPD is strongly associated with smoking but it's not exclusively a smoker's disease. Indoor air pollution from cooking fires, long-term occupational dust exposure, and severe childhood respiratory infections all contribute to its development in people who have never smoked. Many patients are surprised by the diagnosis because they didn't fit the picture they'd formed of who gets it.

What changes the trajectory is catching it early enough that intervention can slow the decline: appropriate inhaler therapy, pulmonary rehabilitation, management of exacerbations before they do cumulative damage. Early spirometry, a 15-minute breathing test, is the tool that makes this possible. Most people who should have had one, haven't.

ILD: The Scarring Most People Don't Know Is Happening

Interstitial lung disease is a group of over 200 conditions in which lung tissue becomes inflamed and then scarred. The scarring reduces the elasticity of the lung, makes gas exchange progressively less efficient, and produces breathlessness that quietly worsens over months and years.

Some forms of ILD are driven by autoimmune conditions, rheumatoid arthritis and scleroderma being among the most common. Some arise from occupational exposures: silica dust, asbestos fibres, bird proteins. Some have no identifiable cause.

That last category, idiopathic pulmonary fibrosis, is the most serious. Median survival from diagnosis sits at three to five years. Antifibrotic drugs now exist that slow progression significantly, but they work better the earlier they're started.

ILD gets missed for years because the breathlessness is gradual enough to be attributed to ageing or deconditioning, and the dry cough it produces gets treated as a post-viral problem or reflux. A high-resolution CT scan reveals the scarring pattern clearly. It tends not to be ordered until someone connects the dots, which often happens late.

Asthma in Adults: Not Quite What You Expect

Adult-onset asthma presents differently from the childhood version most people picture. The wheeze is often less obvious. Cough can be the dominant symptom, sometimes the only one. Breathlessness on exertion may be the presenting complaint with no clear trigger the patient has identified.

Adults with poorly controlled asthma over many years also develop a degree of fixed airflow limitation, where the reversibility that characterises the condition becomes less complete. Managing it well requires accurate diagnosis, identifying triggers the patient may not be aware of, and an inhaler regimen that's actually working at the dose being delivered.

That last point matters more than it sounds. Many adults with asthma are using the right medication through the wrong device, or the right device with the wrong technique. A spacer used correctly delivers substantially more drug to the airways than a pressurised inhaler used incorrectly. That difference shows up in how many symptom days the patient has per week, and in how often they end up in emergency.

The Test Most Patients With Breathing Problems Have Never Had

Spirometry measures how much air the lungs hold and how fast it can be expelled. It takes 15 minutes. It forms the diagnostic foundation for COPD, asthma, and restrictive lung disease, and establishes a baseline future tests can be compared against.

Many people with chronic respiratory conditions have never had one done.

If you've been breathless for months, had a cough lasting more than eight weeks, or been told you have a lung condition without this test being mentioned, that gap is worth closing.

Breathlessness is information your body is sending. It gets harder to act on the longer it's been written off as normal.
